• INDIVIDUAL INCOME TAX RETURN (I.R.S. 1040) for the past two years, including all schedules, W-2(received from employers), 1099 (received from banks, brokerages, etc.) and K-1’s (received fromsmall business corporations, trusts, investment partnerships).
• SIGNED I.R.S FORM 4506 (attached) with social security number furnished.
• CORPORATE INCOME TAX RETURN (I.R.S. Forms 1120 or 1120-S) for the past two years.
• END OF YEAR BANK and/or BROKERAGE STATEMENTS for all accounts for past two years.
• FLORIDA INTANGIBLE TAX RETURNS for the past two years.
• ANY FINANCIAL STATEMENTS RECENTLY SUBMITTED TO MORTGAGE COMPANIES OR BANKS.
